About Us Stewarts Care is a long-established voluntary organisation providing comprehensive community-based services to people with intellectual disabilities in the West Dublin area. We support people with a wide range of support needs across residential settings, day services, supported independent living, job advocacy and vocational training. Purpose of the Post Reporting to the Director of Care, Resident Services, theSocial Care Workerwill be on a permanent full-time contract. The social care worker will support persons moving to or living in independent accommodation to live a life of their choosing using a model of support which builds capacity and promotes independence. In addition, the social care worker will ensure continuity of support when tenant moves to their own homes and ensure it is person centred and quality driven. The social care worker will work as a member of the Independent Living Team and will support each person in conjunction with the wider multidisciplinary team and other relevant agencies. This support will be service user-focused and tailored to their individual needs.The Social Care Worker is to support the PersonInCharge to ensure high quality carein accordance withHIQA standards asrequiredto support strong governance of Designated Centre(s). Key Responsibilities Provide person centred supports for Independent Living (IL) residents. Provide pre independent living assessment and training to determine suitability for programme. Identify deficits during this training and work towards upskilling in same. Assess individual needs, assist in setting goals and creating individual support plans (ISP). Provide weekly support for IL residents in accomplishing the ISP dependent on their needs. To recognise when the ISP requires review based on each residents needs at a particular time. Demonstrate effective time management and ability to manage competing priorities. To promote self-development, independence and individuality of each person in a professional manner. This will include implementation and guidance of resources such as ASDAN, Pathways to Independence and other models relevant to the persons needs. Facilitate and implement a person-centred support plan which would include and promote positive risk taking. Support people to manage their own home, including supporting individuals with maintaining their tenancies and give support and guidance when individuals move to their own home. Support persons to manage their finances giving guidance with bill paying, setting up direct debits, budget management. Provide support in relation to house maintenance, i.e. source maintenance worker, window cleaner, repairmenand budget for same. Support people with personal care ifrequired, which may include shaving, nail care, hair appointments in line with their support plan. Support and encourage people to make choices and life decisions in line with the Assisted Decision Making (Capacity Act) and liaise with legal consult ifrequired. Assistand encourage people to engage in everyday home, social, leisure and work activities, according to the individuals choice. Support people with their health needs asrequired, e.g. taking medication and attending appointments. GP, hospital, etc. Promote health, safety, sexuality and well-being of each person. Work professionally with colleagues, families and external organisations and professionals. Advocate on an individual basis or with organised groups regarding issues related to the needs of IL residents. Participate in the planning and implementation of program social activities. Maintain individual case files according to policies and procedures Participate with the manager in case management review, program evaluation and staff development planning Work and liaise with multi-disciplinary team so as to ensure that the continuity of holistic care for residents is maintained to a high standard. Network with other relevant agencies, community groupsregardingissues and needs of IL residents. Work as part of a team in contributing to the design and further development of the Independent Living programme in line with New Directions Guidelines. Follow the reporting mechanism for all incidents/accidents and record same and be responsible for own administration. Provide written reports asrequiredand have an appreciation of the importance of legal aspects where incidents occur. Communicate with social care leader on a regular daily basis and provide up to date service reports. Maintain a supportive and professional relationship with the residents while respecting the individuals right to privacy. Maintain a professional relationship with residents family where appropriate and required with the residents consent. Maintain record systems in accordance with Stewarts Care policies. Maintain a code of confidentiality and discretion in all dealings with residents, families and staff. Provide crisis intervention as required within their scope of duties. Any other duties which may be assigned from time to time. Experience and Qualifications Candidates must meet the following criteria relevant to the role: Candidates with Social Care Degree level 7 or in progress of same CORU Registration / application in process Excellent communication skills both verbal and non-verbal. Full clean driving license and access to car.
