Identify, develop and participate in community based activities with service users. Identify and facilitate formal and informal training for service users. Ensuring appropriate material is ordered for programme delivery. When requested liaise with parents, advocates, and MDT as appropriate Attend and participate in client review meetings and other relevant meetings as requested. Liaise with outside agencies when requested. Promote and adopt a philosophy of care. Complete all relevant documentation and reports pertaining to service user's files including accident and incident reports. Compile requested reports for the MDT or other parties. Complete risk assessments. Assist service users with personal care as appropriate. Medication Management Be confident in dealing with behaviours that challenge.
