  • Prosper is pleased to invite applications from both internal and external candidates for the role of Admissions, Transfers & Exits (ATE) Coordinator. This role is central to the effective coordination of admissions, transfers and exits across Prospers day, residential and respite services, supporting the organisation to respond to the needs of the individuals we support in a timely, person-centred and compliant manner. The role supports service continuity, capacity management and service planning across the whole organisation. Reporting directly to the CEOs Office, the post holder will work at a cross-organisational level working closely with Operations, Clinical Services, HR, Finance, Quality & Standards and other key stakeholders. Key responsibilities of the role include: Coordinate and manage all admissions, transfers and exits across day, residential and respite services in line with organisational policy and best practice. Act as the central point of contact for ATE activity, liaising with service users, families, HSE referring bodies and internal stakeholders. Lead and coordinate assessment, transition and handover planning, ensuring continuity of care and a person-centred approach. Maintain accurate records, data, waitlists and reports, ensuring timely input across multiple systems and databases. Identify, monitor and communicate service capacity, resourcing implications and changing needs to Senior Management, contributing to service planning and development. Prepare and coordinate reports, returns and governance activity, including HSE submissions, committee coordination and compliance with regulatory and data protection requirements. Required Qualifications & Experience: Level 8 or higher qualification in social care, disability, health, a related area or equivalent professional registration or experience Minimum of 3 years experience working within the social care / disability sector Strong organisational reporting and stakeholder engagement skills Strong IT skills including database and reporting systems Full, clean driving licence and willingness to travel as required. As the role will support Prosper Group, applicants should note the requirement for travel between Meath and Fingal as needed. This is a 39-hour-per-week post. The salary is based on Prospers Grade VI scale. Reporting to the Personal Planning Lead,the Personal Planning Coordinator will support the development and implementation of collaborative high-quality personal planning practices across services in Fingal and Meath. Key responsibilities of the role include: Develop and deliver Prosper Groups personal planning process in line with national disability policy, best practice, and Prospers core values. Work collaboratively with the Personal Planning Lead, managers, frontline staff, and key stakeholders to support a high-quality and consistent personal planning approach. Support and guide frontline staff to implement outcome-focused, evidence-based personal planning, including goal setting and review. Build capacity across services by educating stakeholders and supporting managers and staff through training, mentoring, guided practice, and shadowing. Act as a role model for person-centred, rights-based practice, supporting effective personal plans, active Circles of Support, community inclusion, and improved quality of life outcomes. Required Qualifications & Experience: Level 8 or higher qualification in social care, disability, health, a related area or equivalent professional registration or experience. Minimum of 2 years experience in personal planning within a social care or intellectual disability setting. Strong understanding of person-centred planning and human rightsbased approaches. Ability to work on own initiative with a can-do attitude, demonstrating autonomy and professionalism. Excellent communication, collaboration, and facilitation skills. Full, clean driving licence and willingness to travel as required. CORU registration is required, or actively in progress for applicants who are not yet fully registered. While it is envisaged that the role will be primarily based in Fingal, the Personal Planning Coordinator will be required to travel between Meath and Fingal as needed to support collaboration, consistency, and service development. This is a 39-hour-per-week post. The salary is based on Prospers Social Care Worker grade, with progression to Prospers Grade V.
