As a leading provider of Homecare support you will be joining a well known team with an excellent reputation in homecare. This is an exciting opportunity for a confident care coordinator to continue to grow a well established homecare company, with the full support and guidance of our experienced team. The role will be office based at our offices in Stillorgan, A94 KW44. Must have previous experience. General Duties Reporting to the Care Co-Ordinator lead, Assistant Manager and Clinical lead It is the responsibility of the Care Coordinator to assist the Registered Manager to organise the day to day running of the service provision through the policies, training, supervision and guidance provided by the Company. It is the responsibility of the Care-Coordinator to support the Registered Manager in achieving the aims and objectives of the Company and achieving the level of quality in the service provided to Service Users in accordance with the Company’s Quality Assurance Policy. To assist the Registered Manager in marketing the services provided by the Company and liaising with commissioners, prospective Service Users, health professionals and other agencies connected with training and development of the service. It is the responsibility of the Care coordinators to support service users and healthcare workers by completing their rota to their preference in an appropriate and efficient way. It is the responsibility of the care coordinator to monitor roster queries on a daily basis vis phone and e-mail To ensure communication is prioritised between clients and other staff members To treat clients and staff with respect and deal with concerns in a courteous manner Duties and Responsibilities To be responsible to the Registered Manager for the day-to-day service provision to the Service Users by producing detailed work schedules for Health Care Workers and providing both written and verbal instructions/guidance to them. To be conversant with the HSE requirements and legislation governing the service. To learn new skills, further develop and refresh on existing skills in preparedness for the regulation of Domiciliary Care Services. To be aware of the quality assurance policy of the Company in the provision of a quality service to the Service Users. To always represent the Company in a professional manner, on the telephone, face to face or in written communication. To ensure that telephones are answered promptly, and people are spoken to in a polite and respectful manner. To maintain confidentiality and carry out the Company’s Confidentiality Policy. To report to Team lead for any issues regarding the legislative requirements concerning the services provided and the Staff providing them. To report to the Registered Manager any financial issues that have an impact on the overall budget. To acknowledge and be responsible for all staff and client concerns that come though whilst appropriately managing concerns to avoid further complaints To maintain accurate records and document all correspondence on One Touch to clients and staff To be at least two weeks ahead on rosters To send out rosters from one touch weekly and to always notify clients and carers of any changes one the final roster is issued To participate in weekly meetings with the Team lead and management meetings as required #J-18808-Ljbffr

  • Community Clinical Nurse

    The Community Clinical Nurse will report to the Regional Director and provide high-quality, person-centred clinical nursing care to clients in their own homes and community settings. This role involves assessment, planning, implementation and evaluation of nursing care, working collaboratively with multidisciplinary teams, and ensuring strong clinical governance and compliance with professional standards and organisational policies. Health and Safety Be fully familiar with the company’s obligations under the Safety, Health and Welfare Act and all related regulations. Implement and adhere to all company Health & Safety policies. Report all identified Health & Safety outcomes, risks, and issues to the Registered Manager in a timely manner. Quality Control Support the Registered Manager with the day-to‑day running of the service. Assist in implementing company policies, staff training, supervision, and service guidance. Contribute to achieving the service’s aims, objectives, and overall quality standards, as outlined in the Quality Assurance Policy. Represent the company when liaising with commissioners, prospective clients, healthcare professionals, and partner agencies. Conduct client assessments and create detailed care plans outlining individual needs. Ensure consistent policy compliance, including Health & Safety, safeguarding, and clinical standards. Key Responsibilities Client Assessment & Care Planning Assess client needs and prepare tailored care plans prior to service commencement. Meet with clients and their families to discuss needs and propose care solutions. Engage with acute and community healthcare professionals to ensure appropriate care delivery at home. Conduct care plan reviews at least every six months or sooner if required. Attend MDT meetings with relevant healthcare professionals. Clinical Oversight & Support Conduct caregiver introductions for new clients. Provide training for caregivers on complex care tasks (stoma care, catheter care). Monitor complex cases and manage risks such as medication, stoma, catheter care and safeguarding issues. Escalate clinical risks to regional director, HSE and primary care team where appropriate. Quality Assurance & Record Keeping Complete care plan reviews on clients being discharged following an extended hospital admission or changes in care needs while in community. Follow up with families and healthcare teams as needed. Maintain accurate client documentation in OneTouch, including concerns and call logs. Ensure correct use of the client’s folder and accurate caregiver recording of tasks completed throughout the call etc. Conduct Health & Safety risk assessments during home visits. Provide guidance and support to caregivers regarding client concerns. Manage and mediate client/caregiver issues to ensure positive outcomes. Operational & Governance Responsibilities Lead on clinical governance and oversee all clinical operations. Build and maintain strong relationships with HSE, PHNs, GPs, hospitals, discharge planners, and other members of primary care team. Provide out‑of‑hours support and guidance when required. Coordinate care functions to ensure operational excellence and client wellbeing. Implement internal HIQA‑style mock inspections. Oversee scheduling, clinical care monitoring (OneTouch), quality audits (VIClarity), and digital clinical records. Ensure timely information flow to the HSE. Act as Clinical Governance Lead for the HSE Home Support Service tender. Conduct family education sessions and support client onboarding, especially for private clients. Assist with client profiling and service development.
