    Job Summary: The Care Plan Nurse Administrator at Larissa Lodge Nursing Home is responsible for the comprehensive development, implementation, and continuous enhancement and reviews of individualised, person-centered care plans for all residents. This role involves ensuring that care plans reflect the unique needs and preferences of each resident and adhere to the latest regulatory requirements, including HIQA standards. The Care Plan Nurse Administrator will also play a critical role in training staff, conducting audits, and promoting a culture of continuous improvement within the nursing home. Key Responsibilities: Care Plan Development: Lead and coordinate the creation and ongoing reviews and updates of comprehensive, person-centered care plans for all residents. Ensure care plans are tailored to each resident's health status, personal preferences, and social needs, in alignment with Larissa Lodge's standards, policies and procedures. Collaborate with multidisciplinary teams, including doctors, nurses, social workers, physiotherapists, nominated representatives, and residents to gather relevant information for accurate care planning. Facilitate the involvement of residents and their families in the care planning process to ensure plans are both transparent and resident centered. Incorporate into care plans, appropriate risk assessments, using validated risk assessment tools, clinical risk assessment and positive risk-taking strategies. Ensure that care plans accurately document the resident’s needs and wants, as well as the nursing interventions (or implementations) planned to meet these needs. As part of the resident's health record, the care plan is used to establish continuity of care. Compliance, Quality Assurance, and Auditing: Ensure all care plans comply with statutory regulations and HIQA standards and are updated in accordance with changes in legislation or best practices, the centres policies and procedures. Conduct regular audits of care plans to ensure compliance, identify areas for improvement, and implement corrective actions as needed. Conduct regular care reviews (including observational and documentation audits) to determine staffs’ awareness of resident’s care plans, to ensure that the care provided/ delivered is reflective of the resident’s care plan, goals and objectives, planned interventions, risk assessments and resident’s wishes, desires and rights. Prepare for and actively participate in monthly management meetings, presenting findings from audits and proposing solutions to enhance care plan quality and compliance. Monitor the effectiveness of care plans and modify them as necessary to meet the evolving needs of residents, ensuring the highest standards of care, and that care plans reflect the current needs and status of residents, and reflects e ach residents right to autonomy, self-determination, dignity and privacy. Training and Development: Provide ongoing training and support to nursing and care staff on the development, implementation, and auditing of care plans. Ensure staff are fully informed about regulatory requirements, best practices, and the importance of person-centered care. Disseminate updates on statutory regulations, HIQA standards and industry standards to the care team, ensuring continuous professional development. Resident and Family Engagement: Serve as the primary point of contact for residents and their families regarding care plans, addressing concerns, and ensuring their active participation in the care planning process. Facilitate regular family meetings to discuss and review care plans, ensuring transparency, understanding, and alignment with residents' care needs. Documentation and Record-Keeping: Maintain accurate, detailed, and confidential records of all care plans, ensuring they are easily accessible for staff reference and quality review. Utilise electronic health record systems effectively for the efficient documentation, management and updating of care plans. Continuous Improvement: Identify and implement opportunities for improving care planning processes, with a focus on enhancing resident outcomes and satisfaction. Participate in and lead quality improvement initiatives within the nursing home, fostering a culture of excellence and continuous learning. Qualifications and Experience: Essential: Registered Nurse with valid registration from the Nursing and Midwifery Board of Ireland (NMBI). Minimum of 3 years’ experience in a nursing role, preferably within a residential or nursing home setting. In-depth knowledge of nursing standards, HIQA regulations, and care planning processes and documentation. Strong organisational, leadership, and auditing skills, with the ability to manage multiple priorities and work effectively with multidisciplinary teams. Desirable: Previous experience in a care plan coordination, administrative, or audit role. Postgraduate qualification in gerontology, healthcare management, or a related field. Proficiency in electronic health record systems. Key Competencies: Strong communication and interpersonal skills, with the ability to engage effectively with residents, families, and staff.
