    Support Worker (Outreach Service) x 2 Permanent - 35 hours per week Supervisor/Instructor Salary Scale €35,932 to €51,883 DOE The Role: The Support Worker, along with the Outreach Team will ensure individuals are respected, treated with dignity, supported to make personal choices and decisions on a daily basis. Responsibilities: Take part in the completion of the Assessment of Need for individuals Support the individual to enhance their life by playing a role in ensuring that the goals and objectives are achieved in accordance with their Support Plan Support each individual where appropriate with regards to their health Support each person where appropriate with regards to their personal and intimate care Maintain effective communication with families and relatives, where appropriate. As part of the team at Kare, you will focus on community inclusion and communicating with families. The ideal candidate will have experience working with and demonstrate an understanding of contemporary approaches to positive behavioural support and the principles of community inclusion.
