General Responsibilities The Aftercare Worker will provide structured, person-centred support to families transitioning from homelessness into long-term accommodation. Establish and maintain professional, trusting relationships with service users and families. Provide comprehensive aftercare support to families transitioning from emergency accommodation into long-term housing. Assist families in settling into their new homes, including support with utilities, budgeting, and household management. Deliver guidance on tenancy obligations and promote effective tenancy sustainment practices. Develop, implement, and review individualised, person-centred support plans tailored to each family’s needs. Maintain accurate, timely, and confidential case records in accordance with organisational systems (including PASS) and internal procedures. Support access to essential services, including healthcare, education, childcare, community resources, and employment supports. Advocate on behalf of families with landlords, local authorities, and relevant service providers to ensure appropriate supports are secured. Conduct regular home visits to monitor progress, assess needs, and provide ongoing support. Identify potential risks to tenancy sustainment and implement early intervention strategies. Qualifications Minimum level 6 on the National Framework of Qualification in Social Care / Addiction, Mental Health, Nursing, Psychology. Full driving licence. A minimum of 1 year relevant employment working with a similar client group. Child protection training. Experience External candidates: A minimum of 1 year working in the area of homelessness housing. Awareness of the issues arising for residents/families moving into their own independent tenancy. Experience of managing complex and challenging situations in a client-based service. Knowledge of Children’s First Child Protection. Good time management and organisational skills. Confident in making decisions and prioritising tasks while managing a caseload. Able to assess risk and take appropriate precautions during home visits or community outreach.
